Shropshire, Telford and Wrekin Integrated Care System has an ongoing pilot programme, which enables General Practice to access Advice and Guidance (A&G) on behalf of urology patients.

To understand the impact of the pilot and capture any learning opportunities, we would like to hear your views on what worked well, what could be improved, and how we take this forward in the future.

We have created a short survey which should take no longer than 5 minutes to complete. The results of this survey will help to shape the delivery for A&G across a range of outpatient services.
