The Dynamic Support Register (DSR) is an all-age intervention pathway for people with a learning disability and/or autistic people. The people on the DSR are at risk of going into specialist hospitals (children and adults) or 52-week residential placements (children) if they do not get the right care and treatment in the community.
The DSR enables all agencies involved in commissioning and delivering care and support to provide timely support for those whose mental health or well-being is deteriorating.
Shropshire Telford and Wrekin Integrated Care System (ICS) have commissioned Midlands and Lancashire Commissioning Support Unit (MLSCU) to manage their DSR.
The DSR is for people with a learning disability and/or autistic people who are at risk of admission to a mental health hospital (all ages) or 52-week residential/respite placement (children). Regular panel meetings will assign risk ratings – red, amber, or green. The meetings will focus on people who are most at risk of hospital admission or placement breakdown (rated red or amber).
Appropriate referrals will be for children or adults who, when faced with difficult challenges in their lives, become a risk to themselves or others, which could lead to an admission to specialist/inpatient services.
For children and young people aged 0-25 years, inclusion on the DSR can lead to the allocation of a Keyworker.
Professional referral - by secure email. Contact the Transforming Care Partnerships (TCP) Team if you are unsure how to do this (contact details below).
Self referral – contact the TCP Team by email or phone. They will collect information about you/the person you are referring (child or adult) using an information gathering questionnaire. You can download the questionnaire if you wish to see the questions in advance or complete and send by email.
For a referral to be accepted, a person must either have the capacity to give informed consent or, where capacity is lacking at the time of referral, a best-interest decision has been made. Where appropriate, a person with parental responsibility can consent to the inclusion of a child or young person. The TCP team will be proactive in responding to requests for information to help a person understand, retain and weigh up information about this decision.
Not consenting to inclusion on the DSR will not affect a person’s current support.
The TCP Team does not provide an emergency crisis response service and works 9am – 5pm Monday to Friday.
